Any notification received in less than the time specified will be charged a late cancellation fee for the full balance of the appointment: the flat rate or your insurance reimbursement rate.
Request or notification of rescheduling of appointments must occur directly by phone: 609-346-8740.
All fees regarding medical record requests, certification and research abide by applicable state law.
Generally, costs are: medical record requests - $1.00 per page; medical record certification - $10.00 per certification; and shipping and handling rates will apply. Separately, if no record is found, a medical record research fee will be charged the flat rate of $10.00. Total fees will not exceed the maximum dollar amount set by state law and in certain instances for certain individuals, fees cannot not be imposed.
